For any new patients please bring with you any previous vaccinations or pertinent history/diagnostics. Previous clinics can fax (413-268-3899) or email ([email protected]) records. If we do not have appropriate information we may need to reschedule appointment.
We require ALL pets to be up to date on their rabies vaccination, with proof in the form of a rabies certificate. We do not accept a rabies tag as it does not provide the date or information needed.

We have put together this packet in order to make surgery day as easy and stress-free as possible. It is required you print and inital the last two page if your pet is undergoing surgery.

Canine Pain Questionnaire
Does your cat or dog seem uncomfortable? Are they getting old and having difficulty playing or jumping up on the couch or bed? Many times, our pets will not let us know if they are in pain. As an owner you may notice subtle differences in the way they move, jump or lay down. Any of these changes can be signs of chronic pain.
At the Family Veterinary Center we have developed a fully comprehensive pain clinic. While Dr. Bud does a complete physiological exam and asses even subtle pain responses, Dr. Robin will review and discuss important internal concerns, such as diet and metabolic issues. Drs. Bud and Robin work together to develop a completely personalized treatment plan for your pet!
